You can self-schedule appointments below. If you have difficulty booking online or would prefer to speak with a team member, please call (480) 550-9493, and we will be happy to assist you.

Appointment & Cancellation Policy

A valid credit card is required to reserve an appointment.

New patient consultation fees are collected at the time of booking:

Surgical consultation: $150

Non-surgical consultation: $100

Consultation fees are refundable when an appointment is cancelled at least 48 hours in advance. Cancellations within 48 hours and missed appointments are subject to the full consultation fee.

Consultations rescheduled within 48 hours of the appointment are subject to a $75 late rescheduling fee. If the rescheduled appointment is later cancelled, the applicable consultation cancellation fee will apply.

For all other non-procedure appointments, cancellations within 48 hours and missed appointments are subject to a $100 fee.

We understand that unexpected circumstances arise and will do our best to accommodate appointment changes, subject to availability. By scheduling an appointment, you acknowledge and accept these cancellation and rescheduling policies.
