We have two systems to make scheduling, communication, and paperwork as easy, streamlined and paperless as possible:

1) Jane - Our electronic medical charting and scheduling system. Feel free to book or reschedule appointments online at your convenience through this site.

2) TouchMD - Where new patient forms, consent forms and any photos (such as before and after photos) will be. You will receive an email inviting you to log into TouchMD shortly after booking your first appointment. If you are returning patient and need to log into your TouchMD account, please go to https://patient.touchmd.com.

If you are having difficulty booking your appointment online, please call (480) 550-9493, and we will be happy to assist you.

Please note: A valid credit card is required to secure your appointment time. There is a new patient $150 fee for surgical consultations and a $100 fee for non-surgical consultations collected at the time of booking. This is refundable up until 48 hours prior to the scheduled consultation. For all other appointments, there is a $100 cancellation or no show fee that is non-refundable for any reason for cancellation of your appointment within 48 hours of your scheduled appointment time or no shows. A valid credit card must be on file at the time of scheduling your appointment.

We understand that unexpected circumstances can arise and we will do our best to accommodate any changes to your appointment time, subject to availability. By making an appointment, you acknowledge and accept this no refund policy. If you have any questions or concerns about our policy, please don’t hesitate to contact us.
